Immediate Insight
Clear, objective data on your current cardiovascular health and fitness: measurable findings you and your department can act on, not a checkbox physical.
First Responder & Tactical Athlete Evaluation Program
EMTs, Paramedics, Firefighters, Law Enforcement, Military, and Coast Guard carry a cardiovascular risk due to the burden most careers never ask of anyone. This is a cardiologist-developed evaluation, with cardiologist oversight, built specifically for tactical athletes.
The Risk Is Real
Sudden cardiac events are consistently reported as the leading cause of on-duty death among Firefighters, accounting for roughly 40 to 45 percent of fatalities. Research compiled by the National Lipid Association describes Firefighters as "heroes with hearts at risk," and the same pattern extends across Law Enforcement, EMS, and Military service.
The reason is the job itself: sudden surges from rest to maximal exertion, chronic stress, shift work and interrupted sleep, heat and smoke exposure, and years of irregular meals and recovery. Many on-duty cardiac events occur in responders who had undetected heart disease and passed a basic physical and annual wellness program.
An annual physical built for the average office worker was never designed for this occupation. A cursory screening without a physician on site was not either.
“When lives depend on you, uncertainty is not an option.”
The Evaluation
The specialty labs screen for diabetes, elevated cholesterol, and common deficiencies that can affect performance. The calcium score and CIMT look for subclinical disease years before symptoms. The stress echocardiogram evaluates cardiac structure and function, your blood pressure response to exercise, and your exercise capacity. Importantly, it evaluates for findings that suggest obstructive coronary artery disease. Every result is interpreted by a cardiologist and explained to you face to face.
Beyond the Minimum Standard
Clear, objective data on your current cardiovascular health and fitness: measurable findings you and your department can act on, not a checkbox physical.
Screening for silent risk: cardiovascular risk factors, inflammatory markers, metabolic health, and early indicators of coronary and carotid artery disease, found before they become career-ending or life-threatening events.
A personalized plan from board-certified physicians: prevention strategies, strength and recovery guidance, and a path built to extend both career longevity and the retirement you have earned.
For Departments & Agencies
We work directly with chiefs, department leadership, and agency wellness officers to evaluate personnel as a group: scheduling built around shifts, consistent physician oversight for every member, and documentation your occupational health process can use.
Unlike cursory screening programs, every Hosmane Longevity evaluation is developed by a cardiologist, overseen by a cardiologist, and includes a hands-on history and physical by a board-certified physician. Your people see a doctor, not just a machine.

Commonly Asked Questions
The program provides a comprehensive cardiovascular evaluation with objective, measurable findings and physician documentation. Those findings can support your department's occupational health or fitness-for-duty process, but the program is not itself a formal fitness-for-duty certification. Formal duty determinations remain with your department and its designated occupational medicine provider.
Anyone whose job demands physical readiness under high stress: EMTs and Paramedics, Firefighters, Police Officers and Constables, Corrections Officers, and members of the Military and Coast Guard. The evaluation is also well suited to retired first responders who carried those exposures through a full career.
The job combines sudden surges from rest to maximal exertion, chronic stress, shift work and interrupted sleep, and environmental exposures such as heat and smoke. Sudden cardiac events are consistently reported as the leading cause of on-duty death among firefighters, and many of those events occur in responders with undetected heart disease. The National Lipid Association's review of firefighters and cardiovascular disease is a good place to start reading.
No. This is an elective, cash-pay program that operates outside traditional insurance. Departments and agencies may sponsor evaluations for their personnel, and we offer group arrangements. Due to government insurance contracts, Hosmane Longevity cannot accept patients enrolled in Medicare, Medicare Advantage, or Medicaid.
It evaluates cardiac structure and function, your blood pressure response to exercise, and your exercise capacity with an estimated VO₂ max. It also evaluates for findings that suggest obstructive coronary artery disease, which guides whether any further testing and treatment is needed.
